We are looking for a public health champion who will be embedded with the Maine Center for Disease Control and Prevention (CDC) and will be responsible for organizing, coordinating, and delivering a variety of public health services in the area of HIV prevention and control. The role provides (1) HIV linkage to pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) navigation for clients tested through funded providers; (2) Collects information related to HIV prevention through case investigation, record review, and other methods; (3) provides HIV antibody counseling, testing, and referral to treatment and other social support services; (4) ensures increase uptake and knowledge of PrEP statewide; and (5) maintains case management database for clients referred to PrEP to track progress. Work is performed under administrative direction.

Essential Job Functions:

  • Assist clients referred by funded providers in accessing PrEP/PEP through patient assistance programs, insurance navigation, provider referral, and surveillance monitoring.
  • Prepare and disseminate technical reports for statewide and federal partners.
  • Provide case management for clients referred by state-funded providers to PrEP/PEP and ensure clients are linked to PrEP/PEP, and other support services.
  • Maintain relationships with infectious disease providers, pharmacies, FQHCs, and urgent cards throughout the state to improve HIV prevention outcomes.
  • Collaborate with Infectious Disease Prevention Program staff to drive program priorities.
  • Monitor, implement, and evaluate HIV PrEP outcomes.
  • Perform education and outreach activities and serve as the point of contact for PrEP activities for local and federal agencies, medical providers, and members of the public.
  • Assist in the coordination of HIV outbreak and emergency response.
  • Coordinate and participate in consumer planning groups related to HIV, STD, and viral hepatitis prevention.
  • Collaborate with state-funded organizations, staff, and clinicians to improve HIV morbidity in the state.
  • Participate in and contribute to overall Infectious Disease Program planning and evaluation activities.
  • Review and evaluate integrated HIV, STD, viral hepatitis, and harm reduction data and interventions.
  • Attend relevant meetings, work sessions, and conferences.
  • Contribute to Infectious Disease Surveillance evaluation, performance improvement, outbreak preparedness, and response planning including but not limited to COVID 19
  • Assist in additional duties as reasonably assigned

This position will be a hybrid position with at least two days physically onsite in the Augusta CDC office and will follow the State of Maine Tele-Work policy.

Job requirements
Required:

  • Bachelor’s degree in Public Health, Community Health Education, Public Administration, Education, or one of the Behavioral Sciences and two (2) years’ experience in community or health education program planning, development, implementation, and evaluation. Directly related work experience may be substituted for education on a year-for-year basis.

Preferred candidates will also have:

  • Excellent oral and written communication skills
  • Knowledge of the epidemiological, clinical, laboratory, diagnostic, treatment, and prevention aspects of HIV, STDs, viral hepatitis, and harm reduction
  • Ability to gather, synthesize, and summarize a variety of information for local and federal partners.
  • Experience with data collection and analysis
  • Knowledge of and experience with federal, local, and state policies
  • The ability to establish and maintain effective working relationships with a diverse group of internal and external partners and populations.
  • Computer skills, including knowledge of Excel, PowerPoint, and Word.

And the ability to:

  • Communicate complex health information and policies to many audiences.
  • Analyze data and high attention to detail.
  • Organize, disseminate, and report public health information.
  • Communicate with local and community partners
  • Work independently, including being a “self-starter.”
  • Facilitate groups.
  • Speak publicly.
  • Possess an attitude that fosters a respectful, non-threatening workplace environment.
  • Office programs and ability to learn other programs necessary to perform the job.
