For over 20 years, Healthy Acadia has been building healthier communities and working to make it easier for all people to lead healthy lives.

This position is an integral part of our PROSPER Initiative, a care navigation program open to all families across Washington and Hancock counties. This specific Tribal Community Health Navigator position will focus on outreach and support within the Passamaquoddy community at Sipayik (Pleasant Point). The PROSPER program is available for individuals, children, and families impacted by substance use, as well as for affected others. NAFFA’s programs are considered a "promising practice" by Indian Health Services (IHS) and are designed to strengthen families by helping participants understand the sacredness of their role as parents, build character, and strengthen their identity through their Native American heritage.

Responsibilities: The Tribal Community Health Navigator will work closely with community members and partners to:

  • Become a certified facilitator for the NAFFA "Fatherhood is Sacred" and "Motherhood is Sacred" curriculum and host regular parenting classes for the community
  • Develop and maintain strong, trusting relationships within the Sipayik community
  • Provide in-depth, culturally sensitive navigation and care coordination for clients
  • Connect individuals and families with local and regional health care systems, social services, recovery resources, and other supports
  • Work with the PROSPER team and community partners to build and strengthen referral systems
  • Provide education and outreach about the PROSPER Initiative within the community
  • Work with the evaluation team to obtain feedback from participants
  • Travel throughout the Sipayik community to support clients in person, in addition to offering virtual support

Preferred:

  • Personal or professional knowledge of the recovery process and pathways to recovery
  • Experience in social work, psychology, human services, or a related field is a plus, but extensive life experience is highly valued
