Job Description:
About the Program: We’re looking for a proven, people-first leader with experience managing teams in community-based or nonprofit settings to take on a pivotal leadership role in the success of our Community Health Worker (CHW) program.
CHWs are "natural helpers" — people who share lived experiences with the patients they serve, including background, language, and socioeconomic status. We’re proud of our CHW team, and now we’re looking for a passionate leader to support, guide, and grow their work.
Key Responsibilities:
Lead, support, and coach a team of CHWs serving patients in 6-month cycles
Track and analyze performance data to ensure strong patient outcomes
Collaborate with clinical care teams and community organizations
Facilitate team meetings and educational opportunities patient support groups
Manage project budgets and basic accounting for program expenses
Maintain program compliance, productivity, and participant satisfaction
Oversee data collection and reporting for grant-funded projects
The Ideal Candidate Has:
Strongly Preferred: Demonstrated experience managing teams in a nonprofit, healthcare, or community-based setting.
Strong understanding of healthcare, social services, and public benefits systems
Familiarity with community resources and networks in Central Maine
Proficiency in using data to drive decision-making and improve outcomes
Preferred: Background in clinical social work, case management, or public health
Preferred: Experience with grant writing and program reporting
Excellent communication and interpersonal skills
Bachelor’s degree: Master’s in Social Work (MSW), Public Health (MPH), or related field strongly preferred.
