Job Description:

About the Program: We’re looking for a proven, people-first leader with experience managing teams in community-based or nonprofit settings to take on a pivotal leadership role in the success of our Community Health Worker (CHW) program.

CHWs are "natural helpers" — people who share lived experiences with the patients they serve, including background, language, and socioeconomic status. We’re proud of our CHW team, and now we’re looking for a passionate leader to support, guide, and grow their work.

Key Responsibilities:

  • Lead, support, and coach a team of CHWs serving patients in 6-month cycles

  • Track and analyze performance data to ensure strong patient outcomes

  • Collaborate with clinical care teams and community organizations

  • Facilitate team meetings and educational opportunities patient support groups

  • Manage project budgets and basic accounting for program expenses

  • Maintain program compliance, productivity, and participant satisfaction

  • Oversee data collection and reporting for grant-funded projects

The Ideal Candidate Has:

  • Strongly Preferred: Demonstrated experience managing teams in a nonprofit, healthcare, or community-based setting.

  • Strong understanding of healthcare, social services, and public benefits systems

  • Familiarity with community resources and networks in Central Maine

  • Proficiency in using data to drive decision-making and improve outcomes

  • Preferred: Background in clinical social work, case management, or public health

  • Preferred: Experience with grant writing and program reporting

  • Excellent communication and interpersonal skills

  • Bachelor’s degree: Master’s in Social Work (MSW), Public Health (MPH), or related field strongly preferred.

  • A genuine passion for serving low-income individuals and families
